Liva Healthcare
Liva Healthcare offers free digital health
coaching for people with Type 2 diabetes.
Connecting to a healthcare professional
via an app, the platform sets and monitors
personal goals, including reducing blood
sugar levels, calorie intake and smoking
- as well as increasing activity levels and
sleep.
Simon Pickup, Managing Director
at Liva, explains, "Liva holds the firm
belief that prevention is better than cure.
Though its platform has been proven
to facilitate both. For example, a study
analysing the first diabetes patients
using Liva for more than three months
observed a significant weight reduction of
an average of 3.9kg for diabetes patients
(corresponding to -3.5% of bodyweight
and -1.3 BMI change)."
In the last year Liva has been selected
by the NHS as one of five companies
in the digital field of the NHS Healthier
you: Diabetes Prevention Programme.
GPs can refer patients at risk of Type 2
to the Liva programme in North East
London and Humber, Coast and Vale.
The platform allows GPs to monitor up
to 400 patients at once. With health
professionals responsible for running the
digital coaching sessions, escalating any
issues to senior health professionals and
patients themselves being responsible for
self-monitoring.

Low Carb Program
The Low Carb Program is a 10-week
evidence-based digital solution which
has helped thousands of people with
Type 2 diabetes control their blood
sugar, or even put the condition into
remission. It began with the Diabetes.
co.uk community support forum, which
opened up conversation and real-world
evidence about a low-carb way of eating.
The program outlines a lifestyle low in
carbohydrates, supported with healthy,
unprocessed fats. It includes goalfocused education, access to healthy
recipes, health tracking, behaviour
change mentorship and peer support.
Dr David Unwin helped to develop
the program with Diabetes.co.uk and
the wider diabetes community. He said:
"I'm really interested in empowering
patients by giving them lifestyle choices
in a bid to overturn their Type 2 diabetes.
My passion is, at the point of diagnosis,
offering people a three-month trial of diet
and exercise instead of drugs. Hundreds
of people within my practice are taking
this up. On a weekly basis I'm seeing
patients who are putting their diabetes
into remission without having used any
drugs at all. Not only are we saving the
NHS and the government large amounts
of money on medication, we're literally
transforming their lives at the same time."
Results show that people who
complete the program reduce HbA1c,
lose 7% of their body weight on average,
and as many as one in four people
'reversed' or put their Type 2 diabetes into
remission.

OurPath
As one of the providers of the digital
stream of the National Diabetes Prevention
Programme, OurPath has already been
